Liposuction is the removal of excess fat with a small, straw-like instrument called a cannula that is attached to a suction machine. The fat layer under the skin is converted to a network of tunnels, and the cannula is manipulated within the layers to remove unwanted fatty deposits. An elastic garment is then worn for compression and healing. The result is a resculpting of bulging areas into more attractive contours.

Cloth is a paradoxical raw material for the reconstruction of history. When used as clothing, its most common manifestation, cloth is highly visible, often revealing cultural values and social strata as well as aesthetic preferences. It is also highly mobile, leaving a record of travel, trade, and gift contacts. These factors make it a rich source of historical information. Yet cloth also presents particular challenges as a primary document for historical research. Cloth itself is ephemeral; often used until it wears out, it rarely survives in archaeological contexts. Descriptions of cloth and clothing in travelers accounts or trade records are often colored by the observers own expectations and by the close link between dress and perceptions of morality (or immorality). Colleen E. Kriger undertakes an ambitious task in employing cloth and clothing as the raw material for her investigation of complex histories in West Africa, centered on the lower Niger River region. She succeeds in demonstrating that textiles and clothing can serve as windows onto richly layered African histories. Taking three specific textiles as her starting points, Kriger follows their threads (to use an irresistible metaphor) to linguistic, archaeological, literary, economic, political, technological, and iconographic forms of historical evidence. She draws together these disparate sources to elucidate long histories of innovation and international exchange. The three textiles, all from Nigeria, are an elaborately woven womans wrapper, a pair of embroidered mans trousers, and a wrapper adorned with resist-dyed patterns in indigo dye. The wrappers were collected by the author in the 1970s, and the trousers were collected in 1930. Each textile provides Kriger with a way into broad subjects, including the development of weaving technologies in the region, the interaction between local weavers and regional or transnational influences, the development of indigo resist dyeing techniques, and the impact of colonial policies on African textile production and markets. Each discussion spans chronological and geographical distances, moving from the scant yet tantalizing evidence of centuries-old textiles (beginning with the eleventh-century fabrics found in the Bandiagara region of Mali) to the adaptation of a variety of synthetic fibers and dyes in late twentieth-century Nigeria. Kriger assembles prodigious amounts of information drawn from a range of primary and secondary sources to find connections and offer hypotheses about the development of techniques and styles. She provides tables that summarize the records of British and Dutch textile trading firms, comparisons of textile-related terminology along the West African coast, and surveys of the iconography of adire eleko (the indigo resist developed in and around Lagos in the early twentieth century). Through all of this detailed information, the theme that emerges most strongly-and that represents Krigers most important contribution-is the continuity of change in these ostensibly traditional textile forms: “Textile technology was labor-intensive, but that did not mean that it was static, outmoded, or unprofitable” (178). Thus a long history of change and exchange demonstrates that indigenous textile production has not survived as a holdover, a nostalgic remnant of the past, but rather because it continues to adapt to changing markets.

The purpose of this article is to outline a series of recommendations to aid therapists in developing and integrating spiritual self-disclosure into the therapeutic process. The authors introduce six categories of spiritual self-disclosure by adapting Wells (1994) and Kottlers (2003) categories of self-disclosure. Arguments for and against therapist self-disclosure are offered, along with underlying assumptions inherent to the use of self-disclosures that incorporate spiritual themes. Assumptions and recommendations for therapists that may facilitate clinically relevant, culturally sensitive, and ethical implementation of spiritual self-disclosure are offered. Case examples highlighting how therapists can integrate spiritual self-disclosures into clinical practice are also provided. Self-disclosure can be broadly defined as “statements that reveal something personal about the therapist” (Hill Knox, 2001). The disclosure of personal information by therapists in the course of their work with clients has been a topic of serious deliberation among psychotherapists (e.g., Hill Knox, 2002; Knox, Hess, Petersen, Hill, 1997). Furthermore, when, what, and how much to disclose has been the subject of additional debate, as well as whether self-disclosure is appropriate at all. Many therapists agree, however, that self-disclosure can be a powerful intervention if used properly (Kottler, 2003). Despite the on-going debate among clinicians over therapist self-disclosure, there is a striking dearth of literature on therapeutic disclosures that are spiritual in nature. Spiritual self-disclosure has been defined as a spiritual intervention in which the therapist shares “aspects of his or her own spiritual experience” (Richards Bergin, 2005, p. 287) with the client. To date, the literature has previously addressed spiritual self-disclosure only in abstract, non-precise terms, and most often as a vague intervention strategy (e.g., Richards Bergin, 2005). Thus, the purpose of this article is to integrate spirituality into the discussion surrounding therapist self-disclosure and to provide recommendations for the clinically relevant, culturally sensitive, and, ethical use of spiritual self-disclosure. Our goal for this article is to demonstrate how therapists might utilize spiritual self-disclosure to normalize client experience, offer encouragement, provide modeling, or convey a message or information without imposing their beliefs on clients.

Nearly all bridge decks are constructed of concrete. And almost all concrete bridge decks contain cracks–lots of cracks. Cracks are the bane of deck longevity. They allow the ingress of salts that cause corrosion of the reinforcing steel, exacerbating concrete cracking and loss of structural capacity. Why does concrete crack? Because as it dries it shrinks, and the shrinkage results in tensile stresses in the concrete, which generally exceed the tensile strength of the concrete as it is setting. Bridge decks are structural elements, hence reinforced. As the concrete dries and shrinks, the shrinkage is restrained by the reinforcing steel inducing compressive forces in the steel; compression in the steel creates tension in the concrete. The tensile strength of concrete is relatively low and takes time and maturity to develop; at early ages the tensile strength is negligible. Attempts to increase concretes tensile strength by increasing the cement content or by adding supplementary cementitious materials, such as silica fume, fail to improve the situation (or may even exacerbate it) because the additional water needed for the supplementary cementitious material increases shrinkage. For portland cement, a water-cement ratio of 0.22 to 0.25 is necessary to hydrate the cement–but at this water content the concrete is unworkable. So most concrete is placed at a w/c of 0.50 or more. The excess water results in concrete with a consistency that can be placed and finished, but the excess water isnt needed for cement hydration, and it therefore bleeds off to the surface, creating water channels. This increases total shrinkage since a significant volume of water is lost. The water channels formed during bleeding then provide a means of ingress for de-icing salts. The solution to this problem is to use shrinkage-compensating concrete, that is, concrete that contains a shrinkage-compensating cement, such as Type K, which causes the concrete to expand slightly. Since the reinforcing steel is bonded to the expanding concrete, it is placed in tension, which causes compression in the concrete. The compressive stress in the concrete negates the tensile stresses caused by shrinkage and prevents cracking. As the concrete dries and shrinks, the compressive stress is relieved, but by this time the concrete has developed some tensile strength that prevents cracking. Proper curing delays the shrinkage, allowing the concrete to develop greater tensile strength and therefore fewer cracks.

A National Music Summit Australian Musical Futures: TOWARDS 2020 was held on 5 September 2008 at the Sydney Conservatorium of Music, organised by the Music Council of Australia. This was the first national event that brought together the myriad of ideas and interests of music organisations and musicians across all parts of the music landscape. One possible health outcome that may be adversely affected by intentional weight loss is immune function. Natural killer (NK) cells, a subset of mononuclear cells, which are an important component of the immune system, lack antigen-specific receptors and are capable of killing cancer cells and virus-infected cells. The aim of a cross-sectional study was to investigate history of intentional weight loss and associations with NK cytotoxicity and lymphocyte subpopulations among healthy, overweight, postmenopausal women. It was hypothesized that repeated intentional weight loss may be associated with lower immune function measures. The subjects (n=114) were a subset of a female study population recruited for an exercise intervention trial. The subjects had to meet the following criteria: postmenopausal, age 50 to 75 years, in good health; sedentary; nonsmoking; and had been at a stable body weight for at least 3 months. Body weight and height was measured. The subjects completed a questionnaire that included the following question: “Within the last 20 years, when you were not pregnant or sick, did you ever lose 10 pounds or more on purpose?” In other questions, women were queried regarding how often they had lost between 120 lb and 19 lb, between 20 lb and 49 lb, 50 lb or more, and how long they had been within 10 lb of their current weight. Demographic information was obtained. Food frequency questionnaires were used to assess dietary intake and alcohol consumption. The use of nutritional supplements was ascertained. Flow cytometry was used to measure NK cytotoxicity at four effector-to-target (E:T) ratios and four enumerating phenotyping lymphocytes. Multiple linear regression was used to investigate associations between weigh loss within the past 20 years and current immune function. Women who reported ever intentionally losing > 10 lbs were found to have lower measured NK cytotoxicity than those who did not (24.7% [+ or -] 12.1% vs. 31.1% [+ or -] 14.7%, respectively, at E:T 25:1; P = .01). Additionally, increasing frequency of previous intentional weight loss was associated with lower NK cytotoxicity (P = .003, trend). As an independent predictor, longer duration of recent weight stability was associated with higher NK cytotoxicity (21.6% [+ or -] 11.9%, 24.4%[+ or -] 11.0%, and 31.9%[+ or -] 14.4% for <2,>2 to <5, and >5 years of weight stability, respectively; P = .0002, trend). The frequency of weight loss episodes was also associated with differences in the number and proportion of NK cells.

To determine how weight loss success due to an RD-led intervention compared with frequent weight monitoring, the researchers examined a weight loss program that included RD-led weight-loss classes and frequent weigh-in visits. They evaluated weight loss on a continuous scale based on attendance of both RD-led weight-loss classes and weigh-in visits that did not include RD interaction. They examined the relative strength of each of these factors on weight-loss success in a population recruited from the surrounding community for a hypocaloric weight-loss study. Eight of the participants for the hypocaloric weight loss intervention had type 1 diabetes mellitus, a condition that makes successful, safe weight loss especially difficult. Therefore, they thought it important to include these subjects in the analysis, and examined whether attendance and weight loss differed from the nondiabetic group. Participants were subjects involved in a larger, yet unpublished clinical weight-loss study. As determined a priori, potential participants in this substudy were the first consecutive 28 subjects enrolled in the larger study. From this group of 28 subjects, two individuals were excluded before analysis. The study subjects were overweight (BMI [calculated as kg/[m.sup.2]] >27.0) men (n= 10) and women (n= 16), some of whom had type 1 diabetes (n=8), recruited to participate in an 11-week weight-loss study at the General Clinical Research Center of the University of Minnesota. At baseline the mean BMI was 33.2 [+ or -] 3.67 and age was 43 [+ or -] 12 years. Subjects with type 1 diabetes had a mean baseline hemoglobin A1c (HbA1c) of 7.2% [+ or -] 1.0%. Volunteers interested in weight loss attended an initial screening visit to determine height, weight, and BMI. Dietary intake and physical activity estimates were made by an RD interview and a clinical assessment of the information was used to estimate energy needs. Total energy expenditure was estimated as the calculated basal energy expenditure adjusted for reported activity and dietary intake. The energy prescription was determined as a 750-kcal deficit from estimated total energy expenditure. Subjects then participated in the 11-week weight-loss phase composed of 33 visits, including 11 weekly 1-hour RD-led weight loss classes and 22 semiweekly non-RD-related weigh-in visits. Class attendance and weigh-in compliance were strongly encouraged but not mandatory. Attendance for each class and weigh-in visit was documented. The RDs developed and implemented an 11-week weight management intervention program. The first class included one-on-one instruction with individualized guidelines for the energy-restricted diet, menu plans, and recommendations for increased physical activity levels. The additional 10 weight-loss classes were designed for a small group setting and covered a number of relevant topics related to short-term and long-term weight loss.
